Sounds to me like the spinnerbait is coming off the snap-swivel. Try tying directly to the line instead of relying on the snap swivel to make things quicker. I always had a really tough time with that. As a novice fisherman I relied heavily on those and now I only use them when fishing with snelled hooks or leaders.
Another trick, if you want to continue to use the snap swivel would be to take a wire twist and wrap it around the bend area of the arm on the spinnerbait and then use some superglue to hold it there. That way there's a lot less room for the snap swivel to move around on the spinnerbait's arm.
