Of all the knots i've tied in my life time, i've found the plain old improved clinch knot to be as good as any. I use that knot for most terminal tackle connections, up to around 50 lb pound line or so. When you get into the heavier fluorocarbon leaders, a clinch knot is incredibly hard to tie and be trustworthy.
for joining leader to main line, I always use an albright knot. Some people like a uni to uni knot, but if you're tieing two lines that have a dramatically different diameter, the albright is the better knot.
The polomar knot is also a great knot, as gdn pointed out. It's easy to tie, and works well for the heavier stuff.
